The Physical Therapist is responsible for evaluating patients referred for service, establishing a plan of care, providing physical therapy services based on the plan of care to obtain short and long term functional treatment outcomes. Physical therapy services are provided for outpatients, skilled nursing residents, medical surgical patients and contracted home healthcare referrals at Mountains Community Hospital.

EDUCATION/EXPERIENCE

Must be a graduate of an American Physical Therapy Association (APTA) accredited program (at least a Bachelor’s Degree, Master’s degree preferred) for a Physical Therapist.

Preferred one to two years experience in outpatient, skilled nursing and/or home healthcare settings.

CERTIFICATES, LICENSES, REGISTRATIONS

Current license or be eligible for licensure as a Physical Therapist issued by the PT Board of the State of California.

Current California Drivers License.

Current Car Insurance.

Current CPR certificate.
